Application
The Application for the School of Correctional Ministries must be submitted to IPM by the following deadlines. Please download the Application for the term you are interested in. If you are a continuing student wishing to register for new sessions, click here.

Medical Records
Three medical documents must be submitted to SCM.
1. Medical History Form - Must be submitted to the Student Health Center, 500 College Ave., Wheaton, IL 60187.
2. Proof of a TB skin test - if you have had this test done in the last calendar year, there is no need to
repeat the test; however, the Student Health Center must receive verification.
3. Proof of Medical Insurance - A copy of your medical insurance card must be submitted to the IPM office.
Academic Documents
All who are applying to SCM must send in their academic records.
If you only have a high school diploma... Have your high school (or GED) send an official transcript of
your work to the Institute of Prison Ministries.
If you have earned your Bachelors degree... You must submit an official transcript from the accredited
institution where you earned your Bachelors degree to the Institute of Prison Ministries.
Check the Frequently Asked Questions page if you are experiencing difficulty obtaining a transcript.

Meal Plan
For those who will be staying on campus during the Summer Residential Courses, meals are included
in the cost for Room and Board. However, if you are commuting you will need to purchase a meal plan.

Course Syllabus
It is your responsibility to download the syllabus for your registered course, as there are assignments that will
need to be completed prior to the beginning of residency courses. You must also purchase the course-required
